How early should you ask for letters of recommendation when applying to grad school?

Start asking at least 4-6 months ahead of time as many professors [depends on subject matter] so they can put it on their agenda/schedule. Follow up each month until you get it.
The earlier the better, but asking them 2-3 months in advance is a good way to go.
As soon as possible, but usually ~2-3 months before your first deadline! Don't give them 2 weeks to do it if you can help it. They won't appreciate it.
